JACKSONVILLE, Fla. -- Basil Thai and Sushi is located on Hendricks Avenue in Jacksonville. The restaurant opened in 2008. Loyal customers love the Thai menu items and the sushi bar. When we visited the restaurant back in August of this year there were 16 critical violations found in the kitchen.

This time the inspector also found 16 critical violations. Many of them were repeat violations such as:
  • No proof of employee training
  • Potentially hazardous food not held at the right temperature
  • Food stored on the floor
  • Rear door needs to be repaired

The manager of Basil Thai and Sushi, Melanie says, "Your last report really hurt my restaurant a lot. Most of our violations are minor - it's nothing big. Turnover with our employees is high so I don't have the training certificates for all of them. I will buy a new door and repair it. I have very loyal customers and I really appreciate their support - they love my food."

 Bonito Grill & Sushi in Fernandina Beach and the Huddle House on South 8th Street also in Fernandina Beach rounded out the top three restaurants with the most number of critical violations.

The following restaurants had zero health violations and get our clean plate award of the week!

  • 95 Cordova - Saint Augustine
  • Spy International Cuisine and Lounge - Saint Augustine
  • Publix - Beaver Street
  • PS - Imeson Road
