Jacksonville, Fla. - Fast Boys Wings is located in a new strip mall on the West side right off of Saint Johns Avenue. The restaurant is known for its wings but the state health inspector says flies were the focus when he was here. Hundreds were seen on the dumpster outside and live flies were seen in the kitchen - with the back door wide open. You can see the full report by clicking here.

We asked the manager Barnie, "Apparently you had quite a fly problem in the back." He replied, "Yep. This is the season we get flies in from the back door."

Barnie was upset to learn the restaurant was at the top of our critical list for violations this week. He says after he returned from vacation last week he fired his kitchen manager for several reasons - including the violations listed in the report. To show us it's all clean now Barnie took us on a tour of the outside and the inside. We did notice several live flies in the kitchen. Barnie replied, "It's hard to keep them out when the door is open." Barnie told us the door was open at the time because there was a delivery for the kitchen. The pest control company also arrived while we were there. Barnie told us they installed fly sprayers in the kitchen to keep the flies out.

Barnie has this message for his customers, "Any time any one of my customers is welcome and I will bring them back to my kitchen. We do the utmost to see that we deliver."

At Monkeys Uncle Tavern in Mandarin, the restaurant was shut down temporarily this week because of roaches. Manager, Steven Slay had nothing to hide - in fact - he said he was expecting us. "I've seen you on TV before and I know you're pretty proactive about this."

We got right to the inspectors emergency report about roaches which you can see by clicking here.

We read him some of the state report, "Dead squished carcasses on the prep table, two dead on the prep table..."

The inspector also found dozens of live roaches found running under the prep tables and dead ones were also found in sticky traps. Slay responded, "We got a freezer that had a roach infestation so they've been breeding for about the last six weeks. They like the heat from the motor so they were up underneath."

We saw a few German roaches crawling on the floor and a one dead one - that's why Slay says they have pest control coming once a week now. The freezer has also been fixed.

The other restaurants at the top of the list this week for violations are:

Tokyo Japanese Steakhouse on Atlantic in Jacksonville had 18 critical violations including:
No manager on duty and the cook not washing his hands correctly

At Bruno's New York Style Pizza in Saint Augustine, the inspector found 17 critical violations including:
Employees touching and making food with their bare hands.
