JACKSONVILLE, Fla. -- Wendy's #58 is located at 3910 University Boulevard in Jacksonville.

Recently, state health inspectors had to close the facility down, so staff could deal with two high-priority violations.

The state inspection report shows dead roaches found under a desk table in the break room.  It also shows live roaches found under coolers in the kitchen.

Wendy's took care of the violations and the restaurant reopened later that day.

Inspectors also spotted at least one roach at the Sushi Factory at 7159 Philips Highway in Jacksonville.

Inspectors found a total of six high-priority violations there, including raw food stored over ready-to-eat food in a cooler.

At the River Pub Italian Grille in Fruitland, inspectors spotted six high-priority violations.

Among them, cooked poultry not reaching 165 degrees and cold food held at high temperatures.  Employees fixed each issue during the inspection.

Four restaurants earned our Clean Plate Award for zero violations.  They are:

Mega's Pizzeria at 5800 Beach Boulevard, Jacksonville.
The Olive Garden at 13040 City Station Drive, Jacksonville.
Carolyn S. Jackson at 103 Cheyenne Avenue, Interlachen.
New Millennium at 1992 NW Bascom Norris Drive, Lake City.

restaurants reports need to check out beach road chicken dinners on 4132 atlantic blvd. its the oldest still established restaurant in Jacksonville but is the most unsanitary place to eat. some of the employees have poor hygiene, like the most important not washing hands. and gloves ,they don't know what gloves are and before your food gets to you it has been touched by two or three employees. I know this because used to work for them years ago and know they been dong business the same way since 1939. They need a reality check. And the kitchen I wont go there but they need a good inspection. In the past I've seen many inspectors get paid off to pass them. just to be known I would not feed an animal out of that kitchen. so restaurant reports need to go to beach road chicken ad see for them selves. They have many skeletons in their closet.
