JACKSONVILLE, Fla. -- Jacksonville Sheriff's Office admits to a major mistake that neighbors say cost a local mother her life.

Police responded to a domestic violence call Monday night at the home of Telia Toaltoan in Riverside. Officers left there without writing a report.

Early Tuesday morning Toaltoan's boyfriend, Prince Sanders allegedly returned to the house and shot her. Sanders left the scene but was later caught by police Tuesday night. Neighbors now say the police didn't do enough to prevent the shooting.

We sat down with former FBI agent and local attorney, Dale Carson to talk about the internal investigation that JSO is now conducting. Watch the video for more.

Your headline is unnessarily inflammatory - the fact is that a report even if filed would not have saved her life. The report snould have been written but she is dead because of one violent drug addict that she let into her life - not because of a missing report.
