Jacksonville, FLA. -  The state health inspector recently issued an emergency at a Saint Augustine Checkers restaurant. It's located at 1000 Ponce De Leon in Saint Augustine. The restaurant was temporarily closed down for three high priority violations found by the inspector including:

Dead roaches
Live roaches, 12 found in back of establishment

Beef, chili and cheese in the walk in cooler was all at a potentially hazardous temperature

The very next day an inspection report shows the roaches were gone but the beef, chili and cheese in the walk in cooler was still an issue.

A Checkers spokesperson sent us this statement:

"The health and safety of our food is our top priority. The issues found during the most recent health inspection were addressed immediately, and we were authorized to reopen by the next morning. We are committed to maintaining a clean restaurant environment."

Here are the other restaurants at the top of the high priority list this week.

At Sumo Sushi in Saint Augustine, seven high priority violations were found there including an employee who didn't wash their hands before putting on gloves to work with food.

At Lewey's Crab House & Soul Food Café there were also seven high priority violations including a license expired for more than 60 days.

The following restaurants had zero violations and earn our Clean Plate Award:

Taste Buds Express: Beaver Street
HCA: Crossing Blvd
Jaxport Inn: Airport Road
Jimmy John's: 510 Airport Circle Drive
Amelia National Clubhouse Grill: Clubhouse Road
Super Food Truck: West Beaver Street
