JACKSONVILLE, Fla. - With 23 violations the McDonald's located at 435 Atlantic Boulevard in Atlantic Beach tops the list this week for the restaurant with the most high priority violations.

Here's what we found in the report filed by a state health inspector:

Our of 23 violations total, five of them were high priority including:
Raw eggs stored next to ice cream mix
An employee who didn't wash their hands before putting on gloves to work with food
Live flies in the kitchen

The owner of this McDonald's, Shawn Kuthan, sent us this statement:

"Operating a safe and clean restaurant, and the health and safety of my customers, are my top priorities.  I have taken immediate action to resolve these concerns."

Other restaurants at the top of the list this week for high priority violations:

Sam's St Johns Seafood #6 on Normandy Boulevard had nineteen documented violations and Daruma Japanese Steakhouse on Beach Boulevard had twelve.

There were no clean plate awards this week.
