All machines are dangerous when operating. Common sense tells us not to put our hands or feet into moving machinery. Blaming a machine for plain stupidity is senseless. You would not stick your limbs into a fan, under a lawn mower, near the fan on a car, or in the tracks of an escalator, or operating conveyor belt. Things happen to those who don't THINK. Sometimes they are called accidents but, most are due to the careless act of self-negligence. I feel sorry for the woman but, what happened was her own fault, plain and simple.

"She reached into a moving conveyor belt to grab a piece of discarded corn husk, when her glove caught in the moving parts." Does anyone see a problem with that?

If there has been other accidents with that machine then they need to get rid of the damn thing, or put in extensive safety procedures. Require the machine to be shut down when cleaning anywhere near it with safety lines in place when it is running to keep people away from it. She was probably doing what she was told to do. How do you replace a fore arm and good hand? Answer is you can't. 75,000 is very small sum for a limb.
