Jacksonville, FLA. - Sushi House No. 5 is located on State Road 200 in Yulee. This week the state health inspector found 22 violations here. Six of them were "high priority," meaning, they could make you sick.
Here's what we found in the state report:Beef and tofu in the kitchen cooler was over-temperature and hazardous.
Quail eggs at the sushi bar had to be thrown out.
Scallops, shrimp and soft-shell crabs were over temperature and had to be moved to the walk-in cooler.
Guo Huang says he has owned the restaurant for six years. Huang says, "I
am sorry, but I promise that the health of the restaurant will be clean and this won't happen again. Sometimes the temperature might be because of the seasons changing. I promise to keep up with that. I have been taking care of restaurants since I was a kid and this won't happen again." Basil Thai and Sushi on Hendricks Avenue in Jacksonville and East Coast Buffet on Regency Square Boulevard in Jacksonville rounded out the top of the list for the restaurants with the most number of critical violations.
