Monday is make-up day at Clanzel T. Brown for those who missed appointment for second dose of COVID-19 vaccine

JACKSONVILLE, Fla. — If you received your first COVID-19 vaccine dose at Clanzel T. Brown, and missed your appointment to get the second dose, tomorrow is your make-up day.

Anyone who missed their appointment for their second shot can return to Clanzel T. Brown Monday between 9 a.m. and 5 p.m. to get their second shot.

No appointment is necessary.

The city asks that individuals bring a photo I.D. and the shot record card they received at their first visit.

The Department of Health Vaccine Screening and Consent forms will be available on site; however, you are encouraged to fill it out before you arrive to reduce your wait time. A copy of the form can be found here.
