• Head-on crash kills 5, injures two children in Clay County

CLAY COUNTY, Fla. - Two children are the only survivors in a deadly crash that killed five people in Clay County Tuesday afternoon.

The head-on crash happened on State Road 21 near Camp Blanding.

Florida Highway Patrol says six people were in a 1999 Toyota Camry traveling northbound on SR 21.

For unknown reasons, the Camry crossed the center double lines and hit a 2014 Nissan Altima head-on.

The Altima was being driven by Chelsea Lynn Johnson. Johnson, who was alone in the car, died.

A family friend told Action News that Johnson was on her way home from her husband’s graduation.

Her husband works with the Putnam County Sheriff’s Office.

Four adults in the other car were also killed. But investigators say none of them had IDs on them.

The two kids were also in that car and are in stable but critical condition at UF Health in Jacksonville.

FHP says they plan to release new information Wednesday.
