Restaurant Report: Zoes Kitchen, Clark's Fish Camp, others receive multiple health violations

The restaurant, Zoes Kitchen, located at 10422 San Jose Boulevard in Jacksonville received nine violations. Three of the violations listed were considered high priority.

  • Raw animal food stored over "ready to eat" food
  • Potentially hazardous food temperatures

Zoes Kitchen issued the following statement:

"We have taken immediate actions to correct the issues at this restaurant, including replacing kitchen equipment and are actively working with our team members to reinforce our training standards to ensure this does not happen in the future."

Nordstrom Bazille, located at 4835 Town Crossing Drive in Jacksonville received 13 violations.

Inspectors found:

  • Potentially hazardous food temperatures
  • cutting board stained/soiled
  • Menu does not identify dressings that contain raw ingredients

Gator's Dockside, located at 8650 Baymeadows Road in Jacksonville received 15 violations.

Inspectors found:

  • Three live flies
  • Potentially hazardous food temperatures
  • Mold like substance in ice machine

Clark's Fish Camp, located at 12903 Hood Landing Road in Jacksonville received 18 violations.

Inspectors found:

  • Five small flying insects
  • Potentially hazardous food temperatures
  • Manager lacking proof of food manager certification
