Deputies said he was last seen on Tuesday, Dec. 21 leaving Dayspring Senior Living at 553600 U.S. Highway 1 near Conner Cutoff. (Nassau County Sheriff's Office)
He is 5′5″ and was last seen wearing black pants, black shoes and a blue shirt with white writing. (Nassau County Sheriff's Office)
