Jacksonville nurse's license restricted by state after reports of forging prescriptions

JACKSONVILLE, Fla. — The Florida Department of Health issued an order of emergency restriction of license for a Jacksonville nurse after reports that she forged a doctor’s signature on prescription pads to obtain Adderall for personal use.

The ERO was issued Monday for registered nurse Kimberly Crosby who used to work for North Florida OBGYN in Jacksonville.

State records show Crosby used it to get 60 Adderall pills on at least six different occasions from January through May of this year.

The report said staff at North Florida OBGYN confronted Crosby in May and she “admitted to forging the prescription.”

A spokesperson for North Florida OBGYN said Crosby was immediately fired after they learned about the fraud claims and confronted Crosby.

Crosby’s Florida nursing license was first issued in 1997 according to the health department.

The ERO report said that Crosby took part in “deceptive conduct” and lacks “good moral character.”

The ERO prohibits Crosby from practicing in any setting where she has access to prescription pads or controlled substances.

Action News Jax tried speaking with Crosby about the disciplinary action taken against her but there was no answer at her Nassau County home Tuesday.

Action News Jax did not find any arrest records for Crosby in Jacksonville or Nassau County related to the fraud claims.
