JACKSONVILLE, Fla. — The Pfizer, Moderna and Johnson & Johnson COVID-19 vaccines received emergency use authorization in the United States from the Food and Drug Administration.

Floridians and Georgians age 18 and older are eligible to get a COVID-19 vaccine. At this time, the only vaccine approved for 16 and 17-year-olds is the Pfizer vaccine.

In many cases, the appointments to get the vaccine need to be made online. Here are the links you need to utilize to make appointments or request the vaccine.

(NOTE: There was a brief pause in use of the Johnson & Johnson vaccine but as of April 23, use of the vaccine has resumed in most areas after Health officials lifted the pause on the Johnson & Johnson vaccine)

CVS

Select CVS stores in Florida and Georgia are offering the COVID-19 vaccine. To learn how to make an appointment at CVS, click here.

PUBLIX

Vaccination appointments are scheduled online only. Appointments cannot be made by calling Publix or the Publix Pharmacy.

WALMART & SAM’S CLUB

WALGREENS

Walgreens is offering the COVID-19 vaccine as part of the Federal Retail Pharmacy Program. To make an appointment at Walgreens click here.

WINN-DIXIE

As of April 29, Southeastern Grocers has resumed the Johnson & Johnson vaccine in all Harveys and Winn-Dixie pharmacies.

Select Winn-Dixie stores are are offering COVID-19 vaccines. Winn-Dixie is encouraging people to check their website to register for an appointment.

OPTIONS FOR VETERANS

  • The North Florida and South Georgia Veterans Health System is offering COVID-19 vaccines to all veterans enrolled for VA health care at one of their local facilities. To schedule an appointment, call 352-548-6000 ext. 103755.
  • The Jacksonville VA Outpatient Clinic has moved its COVID-19 Vaccination clinic to a new off-site leased location at Timuquana Market Place. It’s located at 5150 Timuquana Road Suite 6 in Jacksonville. All vaccinations will continue to be done by appointment only.

NORTHEAST FLORIDA

DUVAL COUNTY

LEGENDS CENTER:

The Pfizer vaccine is being offered at the Legends Center located at 5130 Soutel Drive. City officials said an appointment is NOT necessary. The site is open daily from 9 a.m. to 5 p.m. See vaccination criteria here.

As of May 13, the Legends Center will also offer the Pfizer vaccine to those age 12 and up. A parent or guardian must be present.

PANANA PHARMACY

Panama Pharmacy, located at 7307 N Main St. in Jacksonville, is offering the vaccine. It’s open Monday through Friday from 8:30 a.m. to 7 p.m. and on Saturday from 8:30 a.m. to 1 p.m.

REGENCY SQUARE MALL:

The Regency Square Mall, located at 9501 Arlington Expressway in Jacksonville, began COVID-19 vaccinations on Wednesday, Jan. 20. This is typically an appointment only center, but sometimes, the center announces days where no appointment is needed on social media.

To make an appointment call 1-866-200-3762. (NOTE: Appointments can only be made by calling the number provided, there are no appointments made online).

During your visit, you must have a photo ID that proves that you’re a Florida resident.

As of May 13, Regency will offer the Pfizer vaccine to those age 12 and up. A parent or guardian must be present.

Action News Jax has heard from a lot of seniors who said they have not received a call about scheduling their second dose. The Florida Department of Emergency Management stated no one is due back at Regency at exactly 21 days. In fact, they say you have up to six weeks to get your second dose.

FLORIDA DEPARTMENT OF HEALTH

If you received the first dose of the Moderna vaccine at South Jacksonville Health Services located at 3225 University Boulevard South on April 19, April 22, April 26 or April 29, the second doses are being offered on these days: May 17, May 20, May 24, May 27.

For those that received the first dose of the Moderna at the FSCJ Kept Campus on April 19, second doses will be available on May 14. An appointment is not needed.

CENTRAL HEALTH PLAZA

Second doses of the COVID-19 vaccine is being offered at the Central Health Plaza, located at 515 West 6th Street on the following days: May 18, May 21, May 25 and May 28.

EDWARD WATERS COLLEGE:

Edward Waters College, located at 1658 Kings Road in Jacksonville, is partnering with Agape Family Health to vaccinate seniors and health care workers, on a first-come, first-served basis.

Just walk in and go to one of the tables and you’ll be registered for a vaccine.

Agape is looking to vaccinate people who live in the following five zip codes for now: 32209, 32208, 32210, 32218, and, 32254. Click here for more information.

CELEBRATION CHURCH

Celebration Church is serving as a vaccination site. People can get the Pfizer vaccine from 8:30 a.m. to 5 p.m. Monday through Saturday. It is drive-thru site, It is located at 9555 R G Skinner Pkwy. As of May 13, Celebration Church will also offer the Pfizer vaccine to those age 12 and up. A parent or guardian must be present.

FIRST UNITED METHODIST CHURCH

If you received the first dose of the Pfizer shot at First United Methodist Church of Jacksonville on April 29, officials say the second dose will be given on Thursday, May 20, from 9 a.m. to 7 p.m.

The church is located at 225 East Duval Street.


CLAY COUNTY

The Department of Health said there will be various, pop-up vaccination sites in Clay County beginning April 19 through May 28.

Appointments are not required, but to get the Pfizer vaccine, you must show proof of Florida residency.

The locations and times of the pop-up sites are available at Alert.ClayCountyGov.com.

Those interested in scheduling an appointment for a first dose can book it through the DOH – Clay Bear Run clinic by calling 904-529-2800.

(NOTE: The COVID-19 vaccination POD located at the UF/IFAS Extension Clay County T. Jesse Godbold Building, in Green Cove Springs closed on Friday, May 7. They are no longer offering first doses at this location.)


ST. JOHNS COUNTY

Qualifying individuals who are interested in receiving the COVID-19 vaccine may preregister for an appointment by visiting https://sjcvaccineappointment.sjcfl.us.

Appointments to get the Pfizer vaccine, which is available for people ages 16 and older, can also preregister using the same link.

Those who preregister will receive an automated phone call from (904) 295-3711 with an assigned appointment date and time. Individuals who get a phone call will be provided the opportunity to do one of the following:

  • Accept the assigned appointment.
  • Decline the assigned appointment.
  • Request removal from the registry.
  • Ask for a phone call from a vaccine call center operator.

Individuals who choose to decline the appointment will remain preregistered, and will receive a subsequent phone call with an alternative appointment.

Upon arrival at the vaccination site, appointment holders will be required to provide date of birth, proof of Florida residency, and if under 50 years of age, proof of employment as a healthcare provider with direct patient contact or a Florida Department of Health COVID-19 Determination of Extreme Vulnerability form signed by a physician.

The appointment preregistration system is only for individuals who are receiving their first dose of the COVID-19 vaccine.

COLUMBIA COUNTY

Columbia County and the Florida Department of Health in Columbia County launched a COVID-19 vaccine registration system through Everbridge.

The Health Department is asking all individuals 18 and older that would like a vaccine to sign up.

Second doses will be scheduled through the Everbridge waitlist system. You will receive an automated call, email, or text indicating your appointment time.

NASSAU COUNTY

Nassau County Emergency Management has a call center (904-548-0900) which will be available to assist those that need help registering. The call center will be available 9 a.m. to 5 p.m. Monday through Friday.

APPOINTMENTS are required for ALL individuals in order to receive a vaccine. NO WALK-INS. To be alerted when the next round of vaccine appointments is open, text “NassauSafe” to 888777.


PUTNAM COUNTY

Pre-registration in Putnam County is available by visiting https://myvaccine.fl.gov/, or by calling (866) 201-7193. The website will allow individuals who are eligible to receive the COVID-19 vaccine to be contacted for confirmation of the appointment and provide a date and time. On the website, residents can select Putnam County and submit their contact information. Individuals will be contacted by phone call, text or email and will be assisted in scheduling an appointment.


BAKER COUNTY

The Florida Department of Health Baker County is currently administering the Moderna COVID-19 vaccination the Moderna series is two doses, given one month apart. If you receive one dose of the Moderna vaccine, you should receive a second dose of the same vaccine one month later to complete the vaccination series.

Appointments can be made on a first come-first serve basis by calling the health department at (904) 259-6291 and pressing 1 for COVID. Vaccine candidates will be required to show proof of age, eligibility, and residency at time of vaccination.

BRADFORD & UNION COUNTIES

Pre-registration for Bradford and Union counties is now available by visiting https://myvaccine.fl.gov/, or by calling (866) 201-7193. The website will allow individuals who are eligible to receive the COVID-19 vaccine to be contacted for confirmation of the appointment and provide a date and time. On the website, residents can select Bradford or Union County and submit their contact information. Individuals will be contacted by phone call, text or email and will be assisted in scheduling an appointment.

HOMEBOUND FLORIDA RESIDENTS

Florida Division of Emergency Management has created a registration survey to help facilitate scheduling vaccines for homebound residents.

You can access the registration survey two ways:

1. Send an e-mail to HomeboundVaccine@em.myflorida.com. That will trigger an automatic response with a link to the registration survey.

SOUTHEAST GEORGIA

CAMDEN & GLYNN COUNTIES

The Georgia Department of Public Health’s Coastal Health District, which includes Camden and Glynn counties, says residents can schedule appointments for COVID-19 vaccinations online at chdcovidvax.org.

To be eligible, you must also reside in Georgia (full time or part time) or be employed in the state. You do not have to be vaccinated in your county of residence. Any Georgian can be vaccinated in any county.

The website is for booking your first dose only. Your second dose will be booked at your appointment. If at all possible, you should plan to receive your second dose at the same place you received your first.

Appointments can be booked up to 8 days in advance. If you do not see any available appointments, check again the following day.

Appointments are available on a first come, first served basis for qualifying individuals, and can be scheduled up to 8 days in advance, depending on vaccine availability. The scheduling system is for first dose appointments only. Second dose appointments are booked at the time the first dose is administered.

CHARLTON, WARE, BRANTLEY, PIERCE COUNTIES

An online scheduling portal is available for those eligible to receive a COVID-19 vaccine in the Georgia Department of Public Health Southeast Health District, which includes Charlton, Ware, Brantley and Pierce counties.

Visit https://dph.georgia.gov/covid-vaccine and select the blue box that states “Health Dept. Scheduling” to access the registration system. A questionnaire will be available to assess eligibility. Those who are determined to be eligible will be able to choose your desired vaccination location and time. For those who have issues with the process, call 1-855-473-4374 and an agent will assist you.

Also, the vaccine is being offered to anyone ages 16 and older at the Mall at Waycross located at 2209 Memorial Drive. An adult will need to be present for those age 16 and 17, in order to give consent. The vaccine is free.

KROGER

Kroger Pharmacy, which has a location at 1606 Memorial Drive in Waycross, is offering the vaccine to eligible parties. To determine eligibility and make an appointment, click here.
