Enter to win an iPad by taking Nassau Community Health Assessment survey

NASSAU COUNTY, Fla. — Department of Health-Nassau is conducting a Community Health Assessment (CHA) to identify factors that affect the health of the community and to determine available resources to address those factors.

The information gathered is also used to prioritize issues and make decisions on programs and health improvement plans that serve the county.

The Department of Health is asking residents of the county to consider taking the community health survey. The survey has about 29 questions (not all questions may pertain to certain individuals and you able to skip them) and only takes a few minutes to complete.

Take the survey today and enter to win a chance for an iPad.

All responses are important, fully confidential and anonymous. Individuals may provide their email address once they’ve completed the survey. Those that provide their emails will be entered into a drawing to win an iPad.
