Jacksonville: Here’s how to get your second COVID-19 vaccine dose at Mandarin, Lane Wiley senior centers

JACKSONVILLE, Fla. — If you received the your first dose of the COVID-19 vaccine at one of the city-managed vaccination sites, the second dose will be available next week, Mayor Lenny Curry announced Thursday.

Beginning on Monday, Feb. 1, residents can return to the vaccination site (Mandarin or Lane Wiley senior centers) where they received their first dose, to get the second one Monday through Friday during the hour associated with the first letter of their last name.

The time slots are:

  • A - D: 9 - 10 a.m.
  • E - H: 10 -11 a.m.
  • I - L: 11 a.m. - 12 p.m.
  • M - P: 12 - 1 p.m.
  • Q - T: 1 - 2 p.m.
  • U - W: 2 - 3 p.m.
  • X - Z: 3 - 4 p.m.

City leaders said those who are not able to visit at their assigned time, should go to the site between 4 and 5 p.m.

Residents must bring identification and the shot record card given to them during the first visit.

Residents are encouraged to bring a completed COVID-19 Vaccination and Consent form to reduce the wait time, but copies of the form will be available at both vaccination sites.

The form is provide on the city’s website at COJ.net/CovidVaccineForm.

Residents will receive reminder phone calls from the City of Jacksonville the day before and day of their appointment. If you need assistance, residents may call the city’s customer care center at 630-CITY (2489).
