Restaurant Report visits Osaka Japanese Steakhouse, Red Elephant Pizza & Grill and Julia's Midtown Cafe

JACKSONVILLE, Fla. -- Osaka Japanese steakhouse is located in a shopping center on Crosshill Boulevard in Orange Park. This week the state health inspector found 22 violations at the restaurant. Ten of the violations are high priority meaning they could make you sick. Take a look at what we found in the state health report:

An employee who didn't wash their hands before putting on gloves to work with food.

An employee who switched from working with raw chicken and then ready to eat veggies and cooked noodles with the same gloves.

Food contact surfaces weren't being properly sanitized.

Osaka manager, Sky told us all of the violations were minor. He took us back into the kitchen and into the main serving area where he says all violations were immediately taken care of.

Sky says the health and safety of customers is their number one priority and it's why they've been going strong for five years now.

He says, "They just like us... so we stay open. I just appreciate everyone for coming."

Here are the other restaurants topping the list with high priority violations this week.

At Red Elephant Pizza & Grill on San Jose Boulevard in Jacksonville there were seven high priority violations including mahi mahi that had not undergone proper parasite destruction.

At Julia's Midtown Café on Lawrence Boulevard in Keystone Heights there were six high priority violations including 32 rodent droppings found in the dough room. They were thrown away during the inspection.

The following restaurants had zero violations and earn our clean plate award:
